NOTE: death certificates are issued by office 0.18.
Declaration of death
The declaration is made in office 0.18 when the person dies or is found dead in Schaerbeek. It is generally made by the funeral director but this is not compulsory. The funeral director or else the family must then go to office 0.20 with template III C completed by the doctor who certified the death, a certificate stating that the deceased did not wear a pacemaker in the event of cremation and where appropriate, the deceased's identity card, marriage record book and/or a card giving access to the Lawn of Honour or the Veteran's Lawn where applicable.
